Who can make a referral to Advocate?
The short answer is ANYONE. Individuals can refer themselves, or they can be referred by a health care professional, family member, or even a friend. However, a Psychiatrist Referral Form, signed by the individual’s psychiatrist must be received in order to be admitted to our program.
What do I have to do to make a referral to Advocate?
- The preferred, easiest, and fastest way to make a referral is to complete our Online Referral Form located at http://www.advocatesupport.com/referral/referral.html. You can also upload additional documents that you would like us to see, if you have any. Our on-line referral form is available 24 hours a day, 7 days a week. All of the information you provide, and/or upload, is securely encrypted and HIPAA compliant.
- Another option is to download and print our Referral Form which is located at http://www.advocatesupport.com/forms/files/Referral_Form_6-22-09.pdf. Remember to print clearly and when you are done you simply fax the completed form and any attachments to 800-372-0799.
- Finally, you can always just call us at 866-277-2080 x2 and we will be happy to fax or mail a Referral Form to you that you can complete and either fax or mail back to us. Someone is available to speak with you Monday through Friday from 9am till 5PM (excluding Federal holidays), or you can leave us a message any time and we will promptly respond to your request and/or return your call the next business day.
Why do you need a referral from my psychiatrist?
Maryland Regulations require that we obtain a written referral from your psychiatrist for PRP services. We must obtain your diagnosis, your psychiatric history and your current medications in order to determine whether or not you meet our Admission Criteria. Also, it’s important to make sure that your psychiatrist believes that you would benefit from our services and that we coordinate our efforts for your benefit.
What if I don’t have a psychiatrist?
Maryland Regulations require individuals to be under the care of a licensed psychiatrist in order to receive Psychiatric Rehabilitation services. If you need help obtaining a psychiatrist, you can call your insurance company, their phone number should be on the back of your insurance card, or alternatively, you can contact the health department in your county. If you need assistance, you may call Advocate at 866-277-2080 and we will try to locate additional resources to meet your specific needs and situation.
Does Advocate have a day program?
Short answer, no. Advocate provides Community Based Psychiatric Rehabilitation Services. Our Case Workers come to you. We visit our Clients in their homes and in their community.
Does Advocate provide transportation?
Transportation is NOT one of the services offered by Advocate. However, Case Worker’s will, on rare occasions accompany a Client to a medical appointment or other rehabilitation related activity in their community, but Advocate does not require them to do so. Our goal is to help our Clients to develop resources and skills so that they can get to places they want and need to go to on their own, without assistance from us.
